School Bus Complaint Form

Use this form to report traffic violations involving school buses.

You can indicate whether you prefer the offending driver to receive a warning or if you would like charges to be laid.

• If you request a warning: You will not be required to provide a formal statement or attend court. Our officers will follow up directly with the driver.

• If you request charges: An officer may contact you for additional information. You may also be required to provide a formal statement and appear in court to testify as a witness. Please note that the decision to lay charges is at the discretion of the investigating officers, based on the evidence available.

Collection of Information

The personal information contained on this form is collected pursuant to the Community Safety and Policing Act (CSPA) and will be used for the purpose of law enforcement in response to school bus violations.
